The Sponsor Form is available in two formats: PDF (Adobe Acrobat) and DOC (Microsoft Word). PDF format allows you to view or print the form using Adobe Acrobat Reader. Word format allows you to view/edit/save the form in your word processor. You may then print or email the completed form.
